Health Maintenance Organization (HMO) plans

‍Provide benefits through a network of contracted health care providers. Your medical care will be coordinated by your chosen Primary Care Physician (PCP). Should you require the care of specialists, your PCP will refer you to specialists within the HMO network. HMOs may include benefits not available in other types of health plans; however, benefits outside the HMO network are limited to emergency care. You must designate any PCP who participates in the HMO network and who is available to accept you or your family members.
